1) Could 'Banner-Sworn', 'Battle-Scarred', and 'Ritualist' be take at chargen, or are they only available through the special conditions occurring in play?

Banner-Sworn you can take at chargen if the leader or faction is one of your background bonds. Note that the text is "When you mark a bond", not "When you Forge a Bond", giving you the flexibility to make it a chargen bond. Same goes for Blade-Bound and Masked. That said, I'd encourage these as an outcome of play because it makes them more meaningful, but by the rules it's all good.
Ritualist should require a vow and a bond, so it's intended to be taken after chargen as an outgrowth of quests/story.
Battle-Scarred shouldn't be taken unless you become maimed through the course of your adventures. However, people gravitate to it for some reason and it won't shatter the universe if you mark maimed for your character at chargen.

Yep! Ritualist just represents a focus and makes you more effective.
